THE ASSESSMENT PROCESS

A health professional will conduct a 1-hour assessment to evaluate your loved one's physical health, cognitive function, living environment, daily routines, and abilities of the caretaker. An assessment is highly recommended for first-time clients, and existing clients whose medical conditions have changed.

DURING THE ASSESSMENT:

  • Gain a holistic understanding of your loved one's well-being

    • Physical health: Check vital signs, review medical history, and conduct physical examination.
    • Cognitive & mental health: Assess memory, attention, decision-making abilities, mood, and emotional well-being.
    • Home safety: Evaluate safety of living environment and identify potential hazards, such as fall risks.
    • Daily routines: Review medication regime and diet plan. 
    • Primary caretaker: Evaluate caretaker’s ability to provide care and give on-site training if necessary.

after the assessment:

  • Receive personalised insights and recommendations

    • Assessment Report: Summary of diagnosis and potential health risks.
    • Goals: Outcomes that patient and caregivers should work to achieve (e.g improved mobility).
    • Care Plan: A personalised plan outlining additional support or treatment that the patient should seek (e.g physiotherapy).

TYPES OF ASSESSMENTS

A RN assessment focuses on evaluating the patient's physical/mental condition and medical history. It's suitable for individuals who are receiving ongoing medical treatment, have mental health conditions, or suffer from chronic illness, such as cancer.


  • Conduct physical examination (hearing, vision, pain, mobility)
  • Assess psychosocial health 
  • Review diet plan and medication management 
  • Assess medical needs (e.g wound care, catheter changing) 
  • Measure vital signs 

A PT assessment focuses on evaluating the patient's physical function, mobility and rehabilitation needs. It's suitable for individuals who are primarily experiencing difficulties with movement, chronic pain, or recovering from surgery/injury.


  • Assess past or current injuries/surgeries 
  • Test range of motion, strength, flexibility 
  • Assess balance and coordination 
  • Locate areas of pain or discomfort
  • Assess ability to perform activities of daily living 

An OT assessment focuses on evaluating the patient's living environment, ability to perform activities of daily living (ADL), and level of independence. It's suitable for individuals who suffer from physical/cognitive limitations, or have just been discharged from the hospital.


  • Assess living conditions and safety hazards 
  • Provide guidance and training on adaptive equipment
  • Assess physical abilities and limitations
  • Assess ability to perform activities of daily living 
  • Evaluate cognitive function (e.g memory, problem solving)

A ST assessment focuses on evaluating the patient's speech, comprehension, and swallowing abilities. It's suitable for individuals with cognitive conditions, or have impaired communication or swallowing functions due to conditions, such as a dementia.


  • Assess communication and language skills 
  • Evaluate cognitive function (e.g attention, memory)
  • Assess oral-motor skills 
  • Test swallowing ability 
  • Examine caretaker feeding methods

A dietitian's assessment will evaluate the patient's health status, nutritional deficiencies/excesses, and lifestyle habits. It's suitable for individuals who want to prevent diseases or manage chronic conditions (e.g cancer, diabetes) through healthier eating habits.

  • Body composition anlaysis: body mass index, body fat percentage, muscle mass, water retention 
  • Analysis of blood test report (if applicable) 
  • Assessment of nutritional intake
  • Assessment of eating & lifestyle habits 
  • Set nutritional goals & interventions
