Care Assessment
When it comes to care, it can be difficult to determine your loved one’s needs on your own. An initial Evercare Assessment is important to ensure the most appropriate level of care.
Our dedicated Care Manager will come to your home to conduct the assessment, which can take up to two hours. Through this process, our Care Manager can build a personalized Care Team, matching caregivers that best suit your loved one’s needs.

- Bed bound Elderly at home
How does the Evercare Assessment work?
- Our Care Manager will speak with the patient as well as other family members involved
- We will review and consolidate any relevant medical documentation including: medical history and disease history
- Our Care Manager will use a bio-psycho-social approach to conduct clinical observations on the patient’s vital signs, illness and therapeutic profile, as well as adjustments to Activities of Daily Living (ADL)
- After an Assessment, our Care Manager, according to patient’s conditions and situations, will suggest a tailor-made Care Plan for the rehabilitation progress, matching suitable carers to ensure the quality of care
